If you believe you may be eligible for Medicaid support for caregiving, you can apply online or by mail. This process will be completed through your local office.
You can reach out the main office and they can provide a link to the PDF document that you can fill out and send or Fax to DSHS for review.

Information You will need to submit this application:
- A Social Security number.
- Proof of identification.
- Proof of income.
- Documentation of resources (such as bank statements, property tax statements, life insurance).
- Immigration or alien documents.
- Proof of citizenship if you do not Medicare or SSI.
