We conduct long-term care assessments for patients currently residing in adult family homes or those planning to move into one. This process often involves case management, as we help clients navigate the system to find an adult family home that meets their needs. Through our RN delegation program, we collaborate with various adult family homes and can frequently recommend the best fit for each client.
To seek placement in an adult family home, a thorough assessment by a registered nurse is required. This assessment is conducted annually while the client resides in the home.
Although DSHS completes an annual review for clients in need, private-pay patients must independently seek this service. Journey Nursing Service’s long-term placement assessments are comprehensive, thoroughly evaluating the client’s care needs, including their medical history, physical limitations, cognitive limitations, and behavioral limitations.
