Your care is important to us! We are a local Veteran-Owned Agency. We have compiled a list of resources to assist you to access the services that are part of your veteran’s benefit. As the VA system is frequently overwhelmed, they prioritize patients on hospice and those who require high/heavier care needs first. We currently have capacity to assist in your area. Once you have followed the steps below you can request that your request your application be sent to Journey Nursing Services — scheduling@journeynursingservices.com or FAX 425 276-8202 — and we will get services started for you as soon as possible.

In-Home Support Services and Supports include maintenance services such as shopping, meal preparation and light housekeeping. Assistance with personal care needs, such as bathing, dressing, feeding, etc., is also available. Some providers offer social work services to assist in planning for the person’s future care needs and to provide counseling or mental health care.

Steps for Enrollment VA Home & Community Services:

1. The Veteran must be enrolled in VA Healthcare. How To Apply for VA Health Care | Veterans Affairs. If you need assistance, you can call the toll-free hotline at 877 222-8387.

2. The Veteran must get set up with a VA PCP or VA paid PCP through Community Care at VA Puget Sound. Register For Care | VA Puget Sound Health Care | Veterans Affairs. This link also provides onsite information in case they need help enrolling in VA healthcare in person.

3. Once the Veteran has seen a VA PCP or VA Paid PCP in the Community, they can ask the Social Worker assigned to that clinic for a referral to Homemaker Home Health Aide. That Social Worker will screen the Veteran to make sure they meet our qualifying criteria and also for waitlist bypass criteria. This allows them to speak with a professional that may be able to provide community resources or alternative VA resources in the interim, since this program has an extensive waitlist. Each VA Primary Care Clinic has a Social Worker attached. For those being followed by a VA paid community care provider there is an unassigned Social Worker at the Seattle and ALVA campuses which are assigned based on the county where the Veteran resides.
